Faith Formation Registration

You may register online or download a paper pdf Faith Formation Registration Form 2017-18 (40 KB) to complete and turn in to the Faith Formation Office.

Registration is now open for 2017-18 Faith Formation programs.  We begin processing registrations May 7th, 10:00am - 2:00pm in Mullen Commons Vista. The Faith Formation Office is closed the month of July, registrations can be turned into the parish business office.  These registrations will be dated and processed in August. You may register more than one child in Faith Formation programs at the same time online or on the same form. Sacrament preparation is a separate process. A Sacrament registration form is necessary for the Sacraments of Baptism, Reconciliation, Eucharist or Confirmation. First Reconciliation, First Eucharist and Confirmation forms will be sent in June to those who are eligible for this program.

The pdf Diocese of Oakland Child's Medical and Media Permission Form (97 KB) will be collected on our Registration Days. One health form can be used for multiple children in the family – just indicate which specific information belongs to which child.

All children and teens are welcome in our Faith Formation programs and should not be kept away for financial reasons.  The fees indicated on the registration form are collected to ease our expenses.  If payment at this time is a hardship, we will be happy to work it out with you.

Here is information regarding Faith Formation Programs

Preschool meets on Sunday mornings, October to May, during the 9:00 a.m. Liturgy in order that parents may attend Liturgy in a more prayerful manner.  The program is for 3 and 4 year olds and Kindergartners.

Elementary Children in grades 1 – 5 have family based sessions. Monthly family based sessions require an adult to attend with the child. Families meet each month, October through May for two hours then receive resources to continue sharing as a family until the next month. A meal is shared at each session. Families attend that same session time each month. When completing the registration form please mark your first, second and third choices of session.  We cannot guarantee your first choice. Choices are 1st Sunday, noon-2:00pm; 1st Thursday, 6:00-8:00pm; 2nd Tuesday, 4:00-6:00pm, or 3rd Thursday, 6:00-8:00pm. 

Middle School 6th, 7th and 8th Grades sessions are Tuesdays 7:00 - 8:30 p.m. October through the first week of May.   

High School meets on Sundays from 6:30 - 8:00 p.m., October through the first week of May.  Open to all high school students Grades 9 – 12.

Sacrament Preparation Children must be enrolled in a faith formation program for at least one year before beginning the preparation process.  If you meet the criteria please indicate on the Faith Formation registration form that you would like your child to be in sacrament preparation. Information and Sacrament Preparation registration forms will be mailed to you or you can pick them up at the Faith Formation office. First Reconciliation and First Eucharist, students must be 2nd grade or above. Confirmation, students must be 11th grade or above.
